I have gotten this situation and need to straighten it out. Also how do I get the green and blue and red colored display back?

Try using Shift-F to re-stretch the boundary, then if it looks like its going to collapse again during the flattening, stop it (i.e. hold space), pin that section of the boundary (see pinning points video), and restart the flattening.

To get the colors back, select the "Error" option under Display -> Colors. You might have accidentally hit the hotkey that turns on the flattening weights display.
