I would suppose it has to do with what 3D app you are using. 3DS Max has an optimize modifier. (thats what I own so I can't attest to Maya/XSI/Lightwave/etc... |

That's a feature actually ... its called "webbing", and is applied automatically to holes when you use Shift-B on an unflattened shell. You can add and remove webbing with the T and Shift-T hotkeys.
